Catastrophic Grants Guidelines

This application is for UVMC employees (including UVPC) who experience a personal Catastrophic emergency and as a result have a need for financial assistance.

Please read the following information carefully The (ECERF) fund is intended for an UNEXPECTED and recent extreme emergency/catastrophic situation only. This would indicate an individual is in need of emergency funding. (In most cases, if your situation is the result of an ongoing or past event (divorce etc…) it cannot be considered.) However, you may be asked to contact Social Services or other organizations who may be better able to assist you.

  1. Maximum total assistance of not more than $1,500 per employee, per any 5 year period. The request for assistance must involve the employee and/or an immediate family member (spouse or child.) (To be considered for assistance a second time within a 5 year period, the situation must be catastrophic in nature such as death of immediate family member, extended serious/acute illness, house fire, serious accident, etc. Under those circumstances, the committee could waive the 5 year waiting period.)
  2. By completing this form there is no guarantee funds will be available. This will depend largely on the amount of contributions designated annually through the Sharing Plus contributions. Applications will be reviewed by the Committee for approval within 3-4 business days of receipt. Once approved, checks will be requested made payable to vendors only. No cash or check will be given to the employee. The time for checks being mailed to vendors, after approval, will be at least 8-10 business days.

  • Do you have copies of bills attached to this form? If not, this application cannot be reviewed. Credit card bills, collection agency bills and attorney-related bills will not be considered for payment. (Copies can be made in your department or UVMC Library.)
  • Is your request for funding due to an unexpected and recent (last 3 months) event or situation? If not, this application cannot be reviewed.
  • Are you behind in your bills or believe you will be behind on bills due to an ongoing situation? If yes, this application does not apply to you and cannot be reviewed. (This is only for unexpected catastrophic situations.)
